How to customise the feedback form

You can personalise the feedback form to align with your organisation’s branding and user experience. The following customisation options are available:

• Logo – Replace the default logo with your own.
• Logo background – Modify the background colour or add an image.
• Background colour or image – Set a custom background.
• Icons – Change or customise icons (applicable when using the Icon template).
• "How did I do" text – Edit the feedback prompt text.
• Happy, Satisfied, Poor text – Update the label text for feedback options.

1. Selecting a Feedback Template

To customise the feedback form:

• Go to Organisation Settings → Feedback Form

• Choose the template you want to use (e.g., Icon, Radio, or Star).

In this guide, we’ll use the Icon template for demonstration.

2. Customising the Feedback Form

Once you select the Icon template, click Customise to open the editing options.

A. Updating the Logo

1. Click the logo area on the form.
2. A settings menu will appear on the right side of the screen.
3. You can:
• Remove the existing evalyou8 logo and upload a new one.
• Supported formats: PNG, JPG, JPEG (up to 5MB).
• Adjust the logo’s size, height, width, and padding.
• Centralise the logo or align it as needed.

✅ Tip: You can also change the logo background colour or upload a custom background image.

B. Editing the "How did {user} from {org} do?" Text

1. Click the text field directly on the form.
2. Enter the new text you want to display.
3. You can use the following dynamic variables to personalise the feedback request:

• {user} – Displays the user’s first and last name.
• {org} – Displays the organisation name.

⚠️ Note: To use dynamic variables, you must enable the following options in settings:

• Show user’s First Name and Last Name
• Show organisation name

C. Changing Icons

If you’re using the Icon template, you can customise the icons for the feedback options:

• Click any of the feedback icons (Happy, Satisfied, Poor).
• The options panel on the right will update with customisation settings.
• You can:

• Change the icons to circles or other shapes.
• Upload custom icons/images if desired.

3. Saving Changes

Once you’ve made the desired changes:

• Click Save to apply the customisation.
• If you want to reset to the default settings, use the Reset button.
